How To Set Up An Evaluation...

Getting started with All Aboard is simple and easy.  Follow the steps below to start the new patient process. Please call us at 479-445-6800 with any questions.

1. Obtain a prescription from your Primary Care Physician (PCP) for an "evaluation".  If your child is an Arkansas Medicaid recipient, be sure that the prescription is on a "DMS640" form.      *This may be faxed to us at 479-445-6816, mailed, or dropped off in our office by a parent/caregiver.

2. When our staff receives the "script", someone will contact you to obtain additional phone numbers, personal contact information, patient's diagnosis/condition, etc.

3. Our staff will then verify benefit coverage with your insurance company.

4. Our staff will contact you to discuss the benefits quoted, and to set up an evaluation date/time (please allow for 1-2 hours for each evaluation).

5. Click here and print off the New Patient Forms. You may fill these out prior to 1st visit or plan to arrive at least 15 minutes prior to evaluation time in order to begin the paperwork process.

6. Upon arrival for the evaluation, check in with our Office Manager.

7. You and your child will be greeted by a therapist before the testing begins.  For privacy reasons, we ask all parents to remain in the waiting room (rather than come into the therapy area).  If  you would like to watch part of your child’s session, please talk to your therapist.

8. Your therapist will contact you in approximately 1 week with the test results and recommendations.

9. If your child does qualify for therapy, a treatment prescription will be requested by our staff from your PCP.  Please be patient with our staff as we wait on your PCP to return this script.

10. Once this is received in our office, our staff will contact you to set up a "standing appointment".  On average, this will consist of 1-3 times per week, with a 1 hour session for each discipline (occupational, physical, and/or speech therapy).
